The Rajah of Rehab is not the first to tread this path.

According to the Elias Sports Bureau, Carl Pavano is the first Yankee to start on Opening Day after not pitching in the previous season since Hippo Vaughn in 1910.

The Yankees were known as the Highlanders then and played to a 4-4 tie against the Red Sox at Hilltop Park in Washington Heights. The game was called on account of darkness.
